Splitting the craniofacial skeleton has revolutionized the skull base surgery. Tumors like nasopharyngeal angiofibroma, ethamoid malignancies, orbital tumors as well as meningiomas extending the involve infratemporal fossa, skull base, and pterygoid space are effectively tackled. For all these tumors, when required, double fold approach is used i.e. craniotomy from the top and face split from the bottom, so the tumor can be effectively excised without any residue, hemorrhage, neurological deficit and CSF leak. Often neurosurgeon, vascular surgeon and interventional radiologist are teamed up.
